Q: What information will the Doctor need to know?
A: Be prepared to tell the Doctor:

Q: What should I do before my appointment?
A: If you have a facial problem or concern, come without any makeup. Bring in any prescription or nonprescription creams and pills that you have used for this problem. Of course, you will also need to have your insurance card and driver’s license to give to office staff.

Q: What insurances does the office accept?

Q: What are your office hours?
A: Office staff is available to make appointments and give you general information during the following times:

Monday: 9 am - 4 pm
Tuesday: 7 am - 5:45 pm
Wednesday: 9 am - 4 pm
Thursday: 7 am - 5 :45 pm
Friday: 9 am - 4 pm
(Closed on weekends)

Patients are seen: On Mondays, Tuesdays, Thursdays, and Fridays. 7:30 am appointments are available on Tuesdays and Thursdays.

Q: What is the charge for the initial office visit for private patients without insurance?
A: $150 (This includes liquid nitrogen treatment of warts, for example, but not biopsies or removal of skin cancers). Return visits are usually $90.

Q: Do you have "liver spots" on your arms?
A: Dr. Kade has a new chemical peel which smoothes the skin of the arms and lightens the brown spots. It is necessary to do 3-4 peels, each 2-3 weeks apart.

Dr. Kade now offers Botox for wrinkles around the eyes, horizontal lines on the forehead and that permanent frown between the eyebrows. You will still look like you, but better!
